Insurance

We provide super bills for patients who wish to submit claims for reimbursement directly to their insurance provider. 

Insurance Verification

We are happy to verify your insurance for out of network coverage. Please email a photo of the front and back of your insurance card to hello@sohoacubyjen.com, along with your first and last name and date of birth. Once we have your information we will verify your coverage and get back to you within 48hrs. 

What is a Superbill: A superbill is a specialized receipt that includes the information your insurance company may require to review acupuncture services for reimbursement. After your session, we can provide you with a superbill that you may submit directly to your insurance provider for reimbursement. Because every policy is different, reimbursement amounts and eligibility vary by plan.

Payment Policy

  • A credit card is required to hold all appointments, regardless of insurance coverage

  • We accept all major credit cards

  • FSA and HSA cards are welcome and may be used for acupuncture services.

Payment

Full payment is required at the time of service. We accept the following payment methods:
Visa, MasterCard, and American Express (credit or debit).

We also accept HSA and FSA cards, which can be used to purchase treatment packages.

Cancellation Policy

We understand that plans may change, and you might need to cancel or reschedule your appointment.
Please provide at least 24 hours’ notice so we can accommodate other patients.

Appointments canceled or rescheduled with less than 24 hours’ notice
will be charged the full appointment fee.
